Private Medicals – The D4 medical form must be completed to apply for your HGV/LGV/PCV licence – this form is completed by one of our doctors and validated by our surgery stamp and doctor’s signature. The D4 form can be downloaded from the DVLA website site. Full payment is taken upon booking to secure your appointment.
